Negligent to check on my mother throughout the night, resulting in me not being able to sleep for four years. If your loved one cannot access the call button and is not able to get around on their own, move them to another facility or have someone sit with them. Staff is slow to check in on patients.

Back in the summer of 2020, I had to stay here for 2 1/2 weeks to get skilled nursing care and rehabilitation after severely breaking my leg in a fall. I found the atmosphere to be nice, therapeutic, clean, great food, and the staff was so helpful. I recovered fully from my accident and have regained my ability to walk because of the great care that I received here in combination with the care from Centennial Medical Center and home physical therapy after.

My great aunt has stayed here for rehabilitation. The staff she encountered were so polite and nice. They are very consistent with wanting their residents/patients to succeed! On time for every therapy appointment. Very persistent with making sure people are fed, and also make a point to make their residents/patients feel like family. The activity directors were awesome, organized, and kept their guest busy! The administrator, Hunter, and the Administrator Assistant, Bethany, were the most pleasant people. Always smiling, and always eager to help meet your needs. I couldn't have been more thankful my great aunt chose here to attend her rehabilitation.

The Place at the Trace was wonderful! The food , nursing and physical therapy staff were top notch! I couldnot have made it through my recovery without their help!

I have been here for a few weeks now and have had absolutely nothing but complete satisfaction. The staff are all skilled, caring, kind, and compassionate. The facility is clean, the food is awesome . It's no wonder I am recovering well, I am receiving excellent care. Thanks to the NHC PLACE ON THE TRACE . Recommended by me.

This is rated as a “5 star facility” but you wouldn’t believe how they sent my husband home from their week long care upon his voluntary discharge. He asked for a shower for two days but I guess they decided not to waste their time since he was leaving. He stunk to high heaven, he hadn’t shaved all week, his clothes were dirty, etc. They did not take care of his basic needs while in their care! Please beware. I would not give 1 star if given a choice.

My husband was there 3 times in 2019..first two times was good..no complaints..3rd time was a nightmare..he wasn't able to walk without a walker and he had fallen trying to go to the bathroom.he was taken to the hospital after they notified me and ask to take him there..really? Had to clean his room everyday ..there was urine in floors and blood which was not cleaned up and he had hepititis . The nurses was rude. Therapy ppl was great.never again..
